How to Apply
Get a referral from a hospital, polyclinic or General Practitioner that the elderly patient visits. A doctor must say that the elderly person is suffering from an advanced and progressive disease.
What it Costs
The service can cost between $100 and $150 per visit before means-testing. The amount varies based on your household’s income.
