Create Your Medication List
A medication list helps you keep track of your medications and supplements, and how to take them. It also helps you remember why you are taking each of your medications. Follow this step-by-step guide on how to create your own medication list.

Step 1: Write down your personal information and the date
Step 2: Write down your medical conditions
Step 3: Write down your drug and food allergies (if any)
Step 4: List down the medications and supplements you take regularly
Fill in these information about your medication:
- Medication name and strength
- How to take medication
- What the medication is used for
Remember to include:
- All your medication from different doctors or clinics
- Any vitamins, supplements and traditional medicines from pharmacies or medical halls
Step 5: List down the medications and supplements you take only when needed
One example is Paracetamol, which is taken only when needed for pain relief.
Update Your Medication List
Bring your medication list to all appointments
Show your doctor, pharmacist or healthcare team your medication list so that they know what medication and supplements you are taking. This is important as they may not know if you are taking any medication and supplements from other clinics or pharmacies.
You can also take a picture of your medication labels with your mobile phone.
Update your medication list if there are any changes
This will help you stay updated with your medication, so that you can take them correctly. Write down the new date that you have made the changes.
Remember to bring your updated medication list to all your appointments!
