What is Healthier SG?
Healthier SG is a national initiative by the Ministry of Health (MOH) that aims to empower all Singaporeans to take steps towards better health and quality of life in the years to come. Preventive care is key to living long and well. Healthier SG supports you in staying healthy, detecting and managing health issues early - for a healthier you.

Step 1: Register
- Eligible residents will receive an SMS invite from MOH.
- Enrol in Healthier SG via HealthHub mobile app or web portal, or at participating Healthier SG GP Clinics or enrolment stations.
- Choose your preferred Healthier SG-registered clinic.Enrol now
Step 2: See a doctor
- Book your 1st Healthier SG Health Plan consultation.
- Measure your key vitals (i.e., height, weight, blood pressure reading, etc.) and assess your medical history at your enrolled Healthier SG clinic.
- Discuss health goals and co-develop a personalised Health Plan with your Healthier SG doctor (e.g., recommended health screenings, vaccinations, lifestyle adjustments and, for those with chronic conditions, adherence to personalised targets and a follow-up plan).
- Schedule and manage your next annual Health Plan check-in appointment on HealthHub.
Step 3: Participate
- View your Healthier SG Health Plan and keep track of your health goals on HealthHub.
- Participate in recommended health programmes and activities via Healthy 365.
- Follow up at your enrolled clinic during your annual Health Plan check-in.
- Discuss the progress and results of your health goals with your Healthier SG doctor.

Manage your health better through a personalised Health Plan
What is a Health Plan?
A Health Plan is a personalised plan developed by your enrolled Healthier SG doctor based on your state of health and in discussions with you. The purpose of a Health Plan is to guide you towards achieving your health goals or actions that are important for you to take for your long-term health. For those with chronic conditions, your Healthier SG doctor will play a crucial role in managing your care and setting targets for optimal control. As part of your Health Plan, you may be eligible to get a personalised 7-day exercise plan powered by Health Plan AI (beta) tailored to your profile, which includes recommended activities near your preferred locations.
Through a Health Plan, you will be able to:

No personal data will be used to train the AI model behind the Health Plan AI beta feature.
Why do you need an annual Health Plan check-in?
Your Health Plan is a guide that captures your Health Plan consultation with your doctor, and it includes steps you can consider taking to maintain or improve your health. It is recommended that you have at least one annual Health Plan check-in so that you and your doctor are updated on the next steps you should take to maintain or improve your health. The annual Health Plan check-in with your clinic may be done in person or over the phone, e.g., if you have no chronic conditions. For those with one or more chronic conditions, your doctor will recommend at least two visits in the year to better review and manage your condition.
You may schedule a check-in appointment with your clinic, or your enrolled clinic will contact you to arrange your annual Health Plan check-in.

Know your charges and subsidies
Your first Healthier SG Health Plan consultation and subsequent annual Health Plan check-ins to update your Health Plan are fully subsidised. Healthier SG Screening and Vaccinations are also subsidised. In addition, eligible Singapore Citizens who are enrolled in Healthier SG will enjoy special subsidies for screening tests and vaccinations at their enrolled Healthier SG clinic. However, if you consult your doctor for treatment, e.g., for flu or abdominal pain, and agree to receive these services, the clinic's prevailing charges will apply. Investigations performed that are not part of the screening tests and vaccinations are also chargeable, for example, a blood test to monitor a chronic condition.
Help your loved ones enrol in Healthier SG
Caregiver(s) of the residents can help their eligible loved ones enrol in Healthier SG through HealthHub's Health Profiles.
To do so, please ensure the following conditions:
- He/she has received the SMS invitation on Healthier SG from MOH
- You have added him/her as your care recipient (People under my care) on HealthHub
- He/she has enabled full caregiver access for you on their 'Caregiver's Details' page on HealthHub
For Healthier SG, your care recipient needs to first enable full caregiver access in HealthHub before you can proceed to enrol on his/her behalf.
