How to use the HPB vending machine
At the vending machine,
access your Healthy 365 QR code under
“Account” > “My QR code” and scan it using the
machine's scanner.
Step
1:
Open your Healthy 365 app and tap
“Account”

2:
Select “My QR code”.

HPB fitness trackers are available for collection at HPB appointed centres strictly via appointments only*. Appointments can be made through the Healthy 365 app.
Step
1:
Open your Healthy 365 app and under “Quick
Links”, tap on “HPB Fitness Tracker”.
Step
2:
Select “Collect a new HPB Fitness
Tracker”.
Step
3:
Select “Self-collect”.
Step
4:
Select “HPB Appointed Centres”.
Step
5:
Select “Book and manage appointment”.
Step
6:
You will be redirected to the booking site.
Step
7:
On the appointment booking site, select your
preferred location.

How to allow a collection on your behalf
If you are unable to collect your HPB fitness
tracker in person, you may authorise someone (i.e. a proxy) to collect
on your behalf when you book an appointment.
Your proxy must be a Healthy 365 app user and
needs to present your Healthy 365 QR code, as well as the confirmation
SMS or email, for verification at the point of collection.
Appointments can be made through the Healthy
365 app. Please follow the in-app instruction when making an appointment
to have someone collect on your behalf.
Step
1:
Open your Healthy 365 app and under “Quick
Links”, tap on “HPB Fitness Tracker”.
Step
2:
Select “Collect a new HPB Fitness
Tracker”.
Step
3:
Select “Appoint someone to collect on my
behalf”.
Step
4:
Select “Next” to continue.
Step
5:
You will be redirected to the booking site.
Step
6:
On the appointment booking site, select your
preferred location.
Step
7:
Share your QR code with your proxy.
