Managing a chronic disease and the costs involved has been made easier with the introduction of a programme that allows the use of MediSave for chronic diseases.
The Chronic Disease Management Programme (CDMP) was introduced in 2006 to improve the level of care and reduce out-of-pocket expenses for outpatient management of an individual’s chronic conditions. The launch of Healthier SG in 2023 further anchors the management of chronic conditions within the community.
Eligible patients can enjoy subsidies and utilise MediSave at polyclinics and Community Health Assist Scheme (CHAS) GP clinics for these 23 CDMP conditions:
MOH reviews the list of CDMP conditions regularly and takes into consideration different factors, such as disease burden and effectiveness of early intervention in reducing complications.
CHAS, Pioneer Generation (PG) and Merdeka Generation (MG) cardholders can receive CHAS subsidies for services and medications used to treat CDMP conditions, up to the subsidy limits based on one’s card type.
CHAS/PG/MG cardholders enrolled onto Healthier SG can tap on the Healthier SG Chronic Tier Subsidy framework to obtain enhanced subsidies for chronic medications on the Healthier SG Medication list at their enrolled Healthier SG GP clinic.
From Jun 2025, selected medications on the Medication Assistance Fund (MAF) list will be added to the Healthier SG Medication list. Enhanced subsidies will apply for Healthier SG enrollees who hold a CHAS Blue or Orange card.
You can use MediSave at participating clinics to pay for part of your outpatient medical bills for the chronic conditions listed above.
MediSave is claimable for the following services:
Medical consultations if you have been diagnosed with one of the identified chronic conditions under the CDMP and/or its complications.
Relevant investigations (including laboratory and radiological procedures such as X-rays) for the evaluation of the identified chronic conditions and/or its complications.
Medication for the management of the identified conditions and/or their complications.
Allied health services such as nursing, physiotherapy, occupational therapy, speech therapy, dietician and podiatry services that are referred by your doctor for your chronic conditions.
You can use up to $500 for non-complex conditions, or $700 for complex chronic conditions* per annum per patient, under the MediSave500/700 scheme. With the exception of patients seeking treatment at their enrolled HealthierSG clinics, all other patients will see a cash co-payment of 15% that is applicable for their chronic condition treatments.

* Patients with complex chronic conditions are defined as those who (i) received treatment for two or more CDMP conditions in a visit; or (ii) received treatment for at least one CDMP condition with a recognised complication.
