These terms and conditions (“T&Cs”), and all subsequent revisions or amendments made by Health Promotion Board (“HPB”), shall apply to the HPB Loyalty Programme (“Programme”).

Your participation in the Programme signifies your agreement to be bound by all the T&Cs If you do not agree to any of the T&Cs, please do not participate in the Programme. .

In these T&Cs, the words “we”, “our” and “us” refer to HPB.

1. About the Programme

1.1. The Programme is our initiative to incentivise and reward our members when they empower themselves with health knowledge and adopt healthy lifestyle behaviours.

1.2. Registered users of our platforms (“HPB Platforms”) will create a valid Healthpoints Account and be able to earn and accumulate points (“Healthpoints”) by completing qualifying activities on HPB Platforms. HPB Platforms include the Healthy 365 mobile application, HealthHub website (https://www.healthhub.sg/), the HealthHub mobile application as well as other platforms where one can earn Healthpoints from. Healthpoints can beredeemed for rewards such as vouchers, e-vouchers/promo codes, and any other rewards made available on the Healthpoint Rewards Catalogue. (“Rewards”).

2. Eligibility

2.1. To qualify for participation in the Programme, you must:
2.1.1. be a Singapore citizen, a Singapore Permanent Resident, a Singapore employment pass holder, a Singapore student pass holder or a Singapore work permit holder; 2.1.2. have a registered account on any one of the HPB Platforms; and 2.1.3. register for a Healthpoints Account to manage your Healthpoints (“HP Account”) through the relevant HPB Platform.

2.2. If you are an HPB employee or an immediate family member of any HPB employee, you are eligible to participate in this Programme provided that the total value of the Rewards redeemed by you does not exceed S$250 in value per year.

2.3. If you are an employee of any third-party vendors, service providers and/or event organisers providing the Rewards, you are not eligible to participate in the Programme.

2.4. We reserve the right, in our sole and absolute discretion, to discontinue or terminate your participation in the Programme. Once your participation in the Programme has been terminated, all your unused Healthpoints then accrued, shall automatically be cancelled immediately and shall no longer be available for use.

3. HP Account registration

3.1. You are required to register for a HP Account in order to participate in the Programme and to redeem Rewards.

3.2. In signing up for a HP Account with us, you represent and warrant that:
3.2.1. all registration information provided by you is true, accurate, current and complete to the best of your knowledge and belief; and 3.2.2. you will promptly update changes to your registration details.

3.3. We reserve the right to reject your application for a HP Account, suspend your use of or terminate your HP Account in accordance with the T&Cs.

3.4. Upon successful registration of your HP Account, you, as the account holder:
3.4.1. agree to keep your account ID and password confidential and shall not allow other person to use your HP Account; and 3.4.2. shall notify us immediately if you have any reason to believe that the security of your HP Account has been compromised.

3.5. You are solely responsible for any and all activities which occur under your HP Account. We are entitled to treat all activities that occur under your HP Account as having been conducted with your knowledge and authority.

3.6. You acknowledge and agree that we may access your HP Account and its contents as necessary for purposes including but not limited to identifying or resolving technical problems or responding to complaints without prior notice to you.

4. Privacy Policy

4.1. By participating in the Programme and/or registering for a HP Account, you consent to our collection, use, and disclosure (including transfer) of your personal data in accordance with this statement.

4.2. Personal information submitted to the HPB via this portal will be treated in accordance with our Privacy Statement and these terms and conditions. In using the Loyalty Programme platform, you acknowledge and agree to be bound by the Privacy Statement and these Terms and Conditions.

4.3. HPB may amend the Privacy Statement from time to time and will make available the updated Statement on www.healthhub.sg/rewards. The updated Statement will supersede earlier versions and will apply to personal data provided to us previously. Each time you visit the Loyalty Platform, or contact, interact or transact with us, you acknowledge and agree that the latest version of this Statement shall apply to you.

4.4. We may use “cookies”, where a small data file is sent to your browser to store and track information about you when you use our Loyalty Platform. The cookie is used to track information such as number of users and their frequency of use, profile of users and their preferred sites. While this cookie can tell us when you use our Loyalty Platform, it cannot read data off your hard disk or mobile device.

4.5. HPB may collect various types of personal data through the Loyalty Programme, including information that you provide to us through the portal: Your NRIC/FIN Name, Date of Birth, gender, mobile number, email, residential address, and/or other information that you submit to us through the portal.

4.6. HPB may collect, use and disclose the personal data received from you for the purposes listed below and any other related associated purposes (collectively known as the “Purposes”):
4.6.1. Setting up your account with us 4.6.2. Managing your redemptions on the Programme including but not limited to: i. Ensuring delivery of your redemption items ii. Providing status updates on the status of your redemption items, including sending notifications via SMS and email. 4.6.3. Assisting you with enquiries 4.6.4. Obtaining feedback 4.6.5. Accounting, risk and fraud management, and record keeping 4.6.6. Carrying out research, planning and statistical analysis to improve the offering of rewards 4.6.7. Sending you advertisements and information by the Health Promotion Board and its partners 4.6.8. Providing you with necessary reminders for the purpose of redemption

4.7. HPB may use the data collected for:
4.7.1. Sharing with other Government agencies and Public Healthcare Institutions, so as to serve you better, unless such sharing is prohibited by law. 4.7.2. We will not share your personal data with non-Government agencies and non-Public Healthcare Institutions, except where such entities have been authorised by HPB to carry out functions on our behalf for which we have collected your personal information pursuant to these terms and conditions or for other specific Government services.

4.8. You shall be responsible for providing accurate and valid information that is required for the Purposes stated above.

4.9. To safeguard your personal data, all electronic storage and transmission of personal data is secured with appropriate security technologies.

5. Issuance of Healthpoints

5.1. You can earn Healthpoints by completing certain qualifying activities on HPB Platforms. Our issuance of Healthpoints to you may be subject to additional terms and conditions in the relevant HPB Platform.

5.2. We may postpone issuing Healthpoints to you for a reasonable period in the event of unforeseen circumstances, including any delay arising from system issues.

5.3. Each Healthpoint has an expiry date. Any expired and unused Healthpoints shall be automatically forfeited, cancelled and become null and void and no requests for extensions, exchanges, replacement or refund shall be entertained.

5.4. Healthpoints may not be transferred to any other HP Account, or to any third party, and are not exchangeable for cash or credit.

5.5. The status of Healthpoints issued to you will be reflected on the relevant HPB Platform and/or on your HP Account.

5.6. We reserve the right to, at any time, without prior notice or liability to you, revise the amount of Healthpoints awarded to you or correct any errors in the computation of Healthpoints.

6. Redemption of Healthpoints

6.1. The list of available Rewards in the form of a catalogue (“Catalogue”) will be published by us on the relevant HPB Platforms and/or on your HP Account. The Catalogue may be revised by us from time to time without prior notice.

6.2. You may use your Healthpoints to redeem any Reward(s), subject to availability of your selected Reward(s) and whether you have sufficient Healthpoints for your selected Reward(s).

6.3. Your redemption of Healthpoints for any Reward is subject to these T&Cs; terms and conditions contained in the Catalogue; and any other terms and conditions as may be stipulated by the respective participating merchant outlets or third party operators, service providers or suppliers engaged by us to provide Rewards featured under the Programme (“Participating Merchants”). You agree and acknowledge that we and/or any of the Participating Merchants, may at any time vary, modify or amend any of the terms and conditions governing your redemption of Healthpoints, in our absolute discretion, and you shall be bound by such variations and amendments.

6.4. Any Reward(s) once redeemed are not refundable, nor exchangeable for cash, Healthpoints, or for any other Reward.

6.5. We are not liable or responsible whatsoever for the acts or default of any Participating Merchant (including the failure of a Participating Merchant to honour any redemption of Rewards) or in respect of any defect or deficiency in the goods or services provided by the Participating Merchant under this Programme. You agree that we are not an agent of any Participating Merchant and that any disputes about quality or performance of the goods and/or services provided by any Participating Merchant should be resolved directly with that Participating Merchant.

6.6. You may, at any time before your Healthpoints expire, choose to automatically use your expiring Healthpoints to redeem certain Rewards (“Auto-redemption”), including TapForMore (TFM) points and TransitLink e-vouchers. If you choose to do so, we shall have the sole discretion to determine when such Auto-redemption shall take place, provided that such Auto-redemption shall take place when your Healthpoints expire. You also agree that we are not responsible if any Auto-redemption transaction fails for any reason whatsoever. You may choose to have your expiring HP converted into TapForMore (TFM) points or TransitLink e-vouchers automatically, subject to the minimum required number of Healthpoints for each reward option. Option must be selected before HP expiry for auto-redemption to take place. If you do not have an auto-redemption type, your expiring HP will be forfeited

7. HPB vouchers

7.1. We may issue vouchers (“HPB Vouchers”) as one of the Rewards which you may redeem using your Healthpoints. We reserve the right to issue HPB Vouchers in any form as we may determine.

7.2. We may publish a list of Participating Merchants and their outlets who can accept HPB Vouchers in the Catalogue as well as at www.healthhub.sg/rewards. We reserve the right to amend this list from time to time without prior notice.

7.3. Your redemption of HPB Vouchers is subject to the conditions set out below:
7.3.1. All HPB Vouchers are issued in Singapore currency. 7.3.2. Each HPB Voucher is only valid for a single transaction. Any amount remaining in the HPB Voucher after a transaction has been completed will be forfeited. 7.3.3. Any HPB Voucher must be presented to the relevant Participating Merchant(s) who can accept HPB Vouchers and used by 2359hrs on the expiry date indicated on that HPB Voucher. Any expired and unused HPB Vouchers shall be automatically forfeited, cancelled and become null and void and no requests for extensions, exchanges, replacement or refund shall be entertained. 7.3.4. Multiple HPB Vouchers may be used in a single transaction. 7.3.5. In the event that the value of the HPB Voucher(s) used in a single transaction does not cover the full cost of the goods or services, you are solely responsible for paying the balance. 7.3.6. HPB Vouchers cannot be exchanged wholly or partly for cash, Healthpoints, and/or any other Reward. 7.3.7. HPB Vouchers cannot be replaced if lost, defaced, damaged, stolen, expired and/or voided. 7.3.8. Any HPB Voucher which is defaced, damaged or expired will not be accepted for redemption and will be considered null and void.

7.4. Any misuse/tampering/forgery of HPB Vouchers may constitute a criminal offence and the offender will be liable for prosecution.

7.5. Where applicable, the vouchers shall not be used for purchase of gift vouchers, lottery, cigarettes and alcohol.

7.6. To use your HPB Voucher at participating malls, please proceed to the Customer Service Counters to exchange for a mall voucher.

7.7. If we make the HPB Vouchers available in the printed format, you may receive such HPB Vouchers, by postal delivery only. The self-collection option is no longer available from 1 August 2018.
7.7.1 If you have chosen self-collection of your HPB Vouchers (printed format), you will be notified via SMS when your HPB Vouchers are ready for collection. Please collect your HPB Vouchers within 30 days of receiving the SMS. All uncollected HPB Vouchers will be forfeited. 7.7.2 If you have chosen postal delivery of your HPB Vouchers (printed format), you will be notified via SMS when your HPB Vouchers have been mailed out. 7.7.3 HPB Printed Vouchers will no longer be available on the Rewards Catalogue from 1 January 2019.

7.8. You shall be responsible for redeeming your vouchers before the expiry date. Unredeemed vouchers shall be forfeited, and no requests for replacement or refund shall be entertained.

8. Termination / Deactivating of HP Account

8.1. You may terminate or deactivate your HP Account by calling in 1800 223 1313 or writing to HPB_Mailbox@hpb.gov.sg.

8.2. Termination or deactivation of your HP Account shall be deemed as termination of your participation in the Programme and you will not be able to use your outstanding Healthpoints to redeem Rewards. No requests for replacement or refund of your Healthpoints shall be entertained once your HP Account is terminated or deactivated. However, termination or deactivation shall not affect your use of the relevant HPB Platform.

8.3 Once your HP Account is terminated or deactivated, you will only be allowed to sign up for a new HP Account one year after the date of termination or deactivation.

9. General Provisions

9.1. We reserve the right to vary and amend these T&Cs at any time without prior notice.

9.2. Our decision on all matters relating to or in connection with the Programme shall be final and binding on you.

9.3. In these T&Cs, unless the context otherwise requires, the headings are inserted for convenience only and shall not affect the construction of these terms and conditions.

9.4. If any provision of these T&Cs is held to be invalid or unenforceable, then such provision shall (so far as it is invalid or unenforceable) be given no effect and shall be deemed not to be included in these T&Cs but without invalidating any of the remaining provisions of these T&Cs.

9.5. No failure or delay by us in exercising any right or remedy provided by law under or pursuant to these T&Cs shall impair such right or remedy or operate or be construed as a waiver or variation of it or preclude our exercise at any subsequent time and no single or partial exercise of any such right or remedy shall preclude any other or further exercise of it or the exercise of any other right or remedy. Our rights and remedies under or pursuant to these T&Cs are cumulative, may be exercised as often as we consider appropriate and are in addition to our rights and remedies under general law.

9. 6. These T&Cs shall be governed by, and interpreted in accordance with, the laws of Singapore. In respect of any legal action or proceedings arising out of or in connection with these T&Cs or the Programme, you shall irrevocably submit to the jurisdiction of the courts of Singapore. That submission shall not affect our right to institute proceedings in any other jurisdiction.

9.7. Please review these T&Cs periodically for changes and updates. To determine when these T&Cs were last revised, please refer to the Last Revision Date stated at the beginning of these T&Cs.
