It is important that seniors get vaccinated against COVID-19 as they are the most vulnerable group and are at higher risk of becoming seriously ill, if infected. Taking the COVID-19 vaccine helps seniors to protect themselves and their loved ones.

Some seniors are hesitant about vaccination because they may not be familiar with the importance and benefits of vaccination or have concerns about receiving vaccination. Let’s encourage our seniors to be vaccinated and provide the support they need.

Refer our seniors for COVID-19 vaccination today.

For every successful referral of an unvaccinated senior aged 60 years and over, $30 HPB eVouchers will be given to the referrer as a token of appreciation once HPB has verified that the referred senior has completed both doses of the COVID-19 vaccination.

Important Notes:

Pre-registration for the programme will be closed on 23 November 2021. Pre-registration is mandatory criteria for interested referrers to be eligible for the token of appreciation.
Referrers should accompany the senior to the selected Vaccination Centres to receive his/her first dose of vaccination and complete registration for the programme on-ground by 30 November 2021.
All referrers who did not accompany seniors for their first dose at selected Vaccination Centres and complete registration for programme on-ground by 30 November 2021 will not be eligible.
To receive the token of appreciation, referrers should ensure seniors receive their 2nd dose of vaccination no later than 25 January 2022.

STEP 1

PRE-REGISTER TO TAKE PART

If you know of any senior aged 60 years and over who has not received their COVID-19 vaccination, encourage him/her to be vaccinated.

Pledge that you will support them in receiving their vaccination, through the 'Let's Get Our Seniors Vaccinated' programme, and complete this pre-registration form.

A confirmation SMS will be sent to you in the next 1 to 2 days.

Please note that pre-registration for the ‘Let’s Get Our Seniors Vaccinated’ programme 1 to 2 days prior to the date senior receives the first dose of the vaccine is compulsory. There will be no pre-registration on-site and HPB will not be able to complete your registration at the Vaccination Centre without your confirmation SMS.

STEP 2

Bring senior to participating
vaccination centres
and complete registration
for programme

Share your personal vaccination experience and the importance and benefits of COVID-19 vaccination with the senior. Address the senior’s concerns and offer support to accompany him/her to seek a doctor’s advice, if necessary.

Accompany the senior to receive their vaccination at selected Vaccination Centres (refer to the list below) where HPB staff are stationed. Seniors may walk-in to the Vaccination Centre to receive their first dose of vaccination, without any prior appointment needed.

For the senior’s first vaccination appointment, you will be required to approach HPB staff at the Vaccination Centre and show your confirmation SMS and Photo ID (NRIC / FIN) to complete your registration for the ‘Let’s Get Our Seniors Vaccinated’ programme. As part of the registration process, the senior will also be required to provide his/her consent for the referral.

DO:

Pre-register for the programme 1 to 2 days prior to the date of senior’s vaccination.
Accompany seniors for his/her vaccination appointments and support them throughout the process. Referrers will be required to accompany seniors for their first dose to complete the registration process. Referrers who do not do so will not be eligible for the token of appreciation
Ensure senior’s vaccination is done at the selected Vaccination Centres offering this programme (refer to the list above). You will not be eligible for this programme if senior is vaccinated at other locations (e.g. Clinics, Polyclinics, Mobile Vaccination Teams or Home Vaccination Teams).
Check in with the senior after each dose to ensure that he/she is feeling well post-vaccination.

STEP 3

Receive token of appreciation
once senior is fully vaccinated

As a token of appreciation, the referrer will receive $30 HPB eVouchers once HPB verifies that the referrer and referred senior have met the eligibility criteria¹.

The eVouchers will be credited into the referrer’s Healthy 365 account approximately 3 weeks after the senior completes 2 doses of the COVID-19 vaccination at selected Vaccination Centres. You will receive a SMS to inform you once the eVouchers have been credited into your Healthy 365 account. Click here to find out how to access your Healthy 365 account and use your eVouchers.

¹To be eligible for the token of appreciation, the following criteria must be met:

Referrer and referred seniors are both eligible for the programme (Refer to Question 3 in FAQ for more information).
Referrer successfully pre-registered for the programme and received the SMS confirmation prior to the date of senior’s 1st dose of vaccination.
Referrer accompanied senior for his/her first vaccination dose at selected Vaccination Centres and successfully completed the registration process on-ground.
Referred senior received their first dose of vaccine no later than 30 November 2021.
Seniors complete 2 doses of the COVID-19 vaccination at selected Vaccination Centres no later than 25 January 2022, and HPB has verified that the senior has been fully vaccinated.

(I) ABOUT THE PROGRAMME

1. What is this programme about?

The ‘Let’s Get Our Seniors Vaccinated’ programme aims to encourage individuals (“referrer”) aged 17 years and over to refer unvaccinated seniors aged 60 years and over for COVID-19 vaccination and support them in their vaccination journey. With vaccination, seniors will be able to better protect themselves and their loved ones. Referrers are encouraged to support seniors by explaining the importance and benefits of COVID-19 vaccination and assist them to have their concerns addressed by doctors. Referrers are required to accompany the senior for their vaccination and are encouraged to care and support them after each dose.

As a token of appreciation for the time and effort taken to provide care and support for the senior, the referrer will be given $30 HPB eVouchers, which will be credited in the referrer’s Healthy 365 account, for each successful referral.

2. When will this programme start and end?

The ‘Let’s Get Our Seniors Vaccinated’ programme will start from 13 August 2021 and end on 30 November 2021.

3. Who is eligible to sign up for this programme?

You are eligible to participate if you are:

A Singapore citizen, Permanent Resident, Employment Pass holder or Work Permit holder; and
An individual aged 17 years and over at the time of enrolment, based on the calendar year in which the individual was born; and
Not volunteering in HPB’s Health Ambassador Programme

AND the senior you are referring is:

An individual aged 60 years and over at the time of enrolment, based on the calendar year in which the individual was born.
Has not received any doses of the COVID-19 vaccine prior to the date of pre-registering for this programme.

4. Can I participate in this programme if I am not vaccinated?

Yes, you can still participate in this programme even if you are not vaccinated. However, we encourage everyone, especially those who are medically eligible, to receive vaccination. It would be more assuring for the senior to be supported by someone who has undergone COVID-19 vaccination.

(II) PARTICIPATING IN THE 'LET’S GET OUR SENIORS VACCINATED' PROGRAMME

5. Where do I find the list of vaccination centres with the 'Let's Get Our Seniors Vaccinated' programme?

6. What happens if the senior is being registered by more than one referrer?

The unvaccinated senior can only be registered by one referrer. The pre-registration applications will be assessed on a first-come, first-served basis.

7. Can I refer more than one senior for this programme?

Yes, you may refer and receive a token of appreciation for each successful referral, up to a maximum of 10 seniors. Please submit one pre-registration form for each senior referred.

However, if you are able to encourage and support more seniors to be vaccinated, please continue to do so. This is to help seniors to better protect themselves and their loved ones.

8. Who will receive the token of appreciation?

The referrer will receive the token of appreciation if the following requirements are met:

Referrer and referred seniors are both eligible for the programme (refer to Question 3 for eligibility criteria).
Successfully pre-registered for the programme and received the SMS confirmation prior to the date of senior’s first dose of vaccination.
Accompanied senior for his/her first vaccination dose at selected Vaccination Centres and successfully completed the registration process on-ground.
Referred senior received their first dose of vaccine no later than 30 November 2021.
Seniors complete 2 doses of the COVID-19 vaccination at selected Vaccination Centres no later than 25 January 2022, and HPB has verified that the senior has been fully vaccinated.

The token of appreciation will be credited to the referrer’s Healthy 365 account approximately 3 weeks after the senior has fully completed the COVID-19 vaccination.

9. I received an SMS informing me that I am not eligible or not able to continue participation in the programme, even though I accompanied the senior for his/her first vaccination dose and completed the registration process. Why?

Referrers may be rejected from the programme due to one of the following reasons:

Referrer did not successfully pre-register prior to the senior receiving his/her first dose of vaccination.
Senior is below 60 years old.
Referrer has successfully referred 10 seniors prior (maximum number of referrals per person reached).
Senior’s vaccination status could not be determined despite HPB’s best efforts due to incorrect information provided or otherwise.
Senior was already referred by another individual prior.
Senior did not receive both doses of their vaccination at the selected Vaccination Centres.

10. I am unable to accompany senior to the Vaccination Centre for his/her first dose. Can I nominate my friend / family member to accompany senior instead?

It is important that seniors receive their COVID-19 vaccination as soon as possible. If you are not able to accompany senior for their vaccination, you may request that someone else accompany the senior. However, you will not be eligible for the token of appreciation.

We appreciate your effort in encouraging the senior to sign up for the COVID-19 vaccination to protect themselves and their loved ones.

11. Can an individual who was previously diagnosed with COVID-19 be vaccinated?

Individuals who have had an earlier COVID-19 infection, and have recovered, are recommended to receive a single dose of vaccine no earlier than 3 months after the date of their diagnosis if they had not already completed vaccination previously. There is evidence that a single dose would further boost their immunity against COVID-19. If you are unsure, check with the senior’s doctor.

12. The senior I referred was previously diagnosed with COVID-19, and hence is unable to receive 2 doses of the vaccine. Will I be able to receive the token of appreciation?

In this case, if the senior receives a single dose of vaccine no earlier than 3 months after the date of their diagnosis, he/she may be classified as “fully vaccinated”.

As such, you will be eligible for the token of appreciation once senior has been fully vaccinated and all of the following criteria is met:

Referrer and referred seniors are both eligible for the programme (refer to Question 3 for eligibility criteria)
Pre-registered for the programme prior to the date of senior’s vaccination
Accompanied senior for his/her first vaccination dose at selected Vaccination Centres and successfully completed the registration process on-ground
Referred senior received their single dose of vaccine no later than 30 November 2021.

13. How should seniors prepare for their vaccination appointment?

Remember to bring their ID (NRIC / FIN) for both appointments.
Please wear short sleeved attire, or sleeves loose enough to be rolled up.
It is advisable to stay hydrated, not come with an empty stomach, and avoid strenuous exercise.

In addition, seniors should postpone their appointment if they:

Feel unwell
Had a fever in the last 24 hours
Are serving Stay-Home Notice (SHN) or Quarantine Order (QO)
Tested positive for COVID-19 within the last 3 months

(III) TOKEN OF APPRECIATION

14. Where can I find the Token of Appreciation?

The token of appreciation ($30 HPB eVouchers) will be credited to your Healthy 365 account

Download the Healthy 365 app on Apple Store / Google Play.
Create a profile.
Log in to your existing profile with your NRIC and registered phone number if you are an existing user.
Tap on Rewards on the bottom bar.
Tap on My Rewards to view your eVouchers.

15. Can I still receive the token of appreciation if I do not have Healthy 365 app?

No, you will need to download Healthy 365 app in order to receive the eVouchers. A step-by-step guide may be found here.

16. How do I use the HPB eVouchers?

Tap on My Rewards to view your eVouchers.
Tap on the eVoucher you wish to use.
Tap on Use Now to use the eVoucher.
Tap on Next when ready to make payment.

17. What can I use HPB eVouchers for?

The list of participating merchants can be found under the details of the HPB eVoucher on the Healthy 365 app. You will be able to use multiple HPB eVouchers in a single transaction at any of the participating merchants’ outlets. Please note that HPB eVouchers is only valid for 90 days, from the date the vouchers were credited into your account.
