​About the Falls Risk Assessment for Fall Prevention

The Falls Risk Assessment is a pilot initiative supported by the Ministry of Health. Through this programme, we aim to raise the public’s awareness of falls prevention and to help seniors aged 65 and above prevent falls by addressing risk factors of falls early. Alexandra Health System is tasked to assess the elderly population in the North for risks of falls so that actions can be taken to help prevent falls.

If you or your loved ones are aged 65 and above, you can perform the self-screening quiz below first to determine if you require further assessment. You will be assessed by our community nurse and trained staff to identify specific risk areas and receive further advice on how to prevent falls. We will ask you questions on your medical history and assess your physical functional status like vision, hearing, feet condition, etc.

Interventions for Fall Prevention

Our community nurses and trained staff will advise you on the possible interventions to consider if a risk factor has been identified. You can share the assessment results with your family doctor for a referral to the relevant services where necessary (e.g., optometrist for eye tests, audiologist for hearing test, podiatrist for foot issues).

If you or your loved ones are aged 65 and above and say Yes to any of the above, talk to our nurse at any of the Community Nurse Posts below.

The Falls Risk Assessment is fully funded during this one-year pilot period.

If you are referred to a falls and balance clinic or other primary care services, the standard charges apply to private or subsidised patients.
