Diabetes is one of the top 10 causes of death in Singapore and one of the four most common conditions for polyclinic attendances. It is a chronic disease and if not managed well, can deteriorate steadily to cause complications such as blindness, nerve damage, kidney failure, heart disease and limb amputation. Studies show that about half the number of patients already have diabetes-related complications at the time of diagnosis.

HDL-C: A Chronic Disease Management Clinic

The Hypertension, Diabetes, Lipids Clinic (HDL-C) is the first pharmacist-led clinic offering a chronic disease management programme in Singapore. It was established in October 2007 and is now fully-functioning in six polyclinics (Bukit Batok, Choa Chu Kang, Clementi, Jurong, Toa Payoh and Woodlands) with plans for expansion.

What HDL-C Does 

With diabetes, hypertension, stroke and lipid disorders affecting one million Singaporeans, the chronic disease clinic targets the most prevalent chronic conditions in Singapore. The HDL-C pharmacists work in collaboration with doctors, nurses and other allied health professionals to combat the growing need for a chronic disease management programme.

Clinic pharmacists provide care for patients with diabetes, hypertension and dyslipidemia based on referrals made by doctors from the National Healthcare Group (NHG) Polyclinic.

At pharmacist-managed clinical services, the HDL-C pharmacists aim to achieve drug optimisation for all patients through a series of clinical activities described by the acronym CARE PILLS:

Through one-on-one consultation, HDL-C chronic disease clinic pharmacists strive to continue achieving positive clinical outcomes and adding value to patient care.
