Persons with chronic health conditions unable to care for themselves should consider nursing home care for comprehensive medication management services.
Intermediate and Long-Term Care (ILTC) facilities refer to facilities that provide a range of services to support the needs of people who are unable to care for themselves for long period of time due to chronic health conditions. Since November 2003, NHG Pharmacy’s team of trained pharmacists has been providing a comprehensive suite of medication management services to ILTC facilities. NHG Pharmacy was the first to introduce an easy-to-use, medication management system, ConviDose, which helps to make it easier for nursing staff to manage their residents’ medications.
